Remember that this information should only be sent if the potential client does not budge on this request. But, go ahead and fill out the information and the potential client will receive an email with more information immediately. You can even do this with them while you're on the phone with them. But, remember to put get to schedule a call in the meantime.
Also, don't forget to follow up with them the next day, if you have not scheduled an appointment for them.
